SECTION A: Applicant details

If you are completing this form as a proxy on behalf of a student, select applicant type For another person. You will need to provide the name, contact information, nationality, date of birth of the student you are applying for.
SECTION B: Course information
Please provide details of your university/institution and course, including course name, duration, mode of study and which academic year of your course you are applying for a grant for.
SECTION C: Education and employment information
Please provide details of your recent education history and any relevant employment or voluntary roles. You can upload your CV (max 2 pages) or add the information manually using the tables provided in the application form. If you are uploading only a CV, please make sure it also includes your education history.
When telling us about your recent education history, we are interested in your undergraduate degree or other recent, relevant studies or professional qualifications. We do not need to know about your GCSE results unless you think they are particularly relevant to your application.
When telling us about your relevant employment and voluntary roles, we would like to know about your most recent roles which are related to your choice of study and career ambitions. You do not need to list every job or role you have ever had.

- Confirmation of your disability
- Confirmation of your study place
- Invoices, quotes or cost estimates for your grant request
If applicable, you will also need to upload:
- Your full Disabled Students Allowance (DSA) Needs Assessment Report. Please ensure this is not password protected or provide us with the password.
- Your DSA approval or rejection letter. Please ensure this is not password protected or provide us with the password.
- Your Learning Support Agreement (LSA), if you have one
- Your Education and Health Care Plan (EHCP), if you have one
- Optional academic reference, if you have one
If you are submitting your DSA Needs Assessment Report as your disability evidence in 15a, you do not need to re-submit it again in 15d “A full copy of your Disabled Students’ Allowance (DSA) Needs Assessment Report (NAR)”.
You can use question 15i “Other supporting documents” to submit any documents which do not naturally fit into one of the other categories.
