This page lists the additional mobility related questions you'll be asked. If your application is succesful, the Occupational Therapist will use these questions to help complete their assessment of your equipment needs.
- What is your height (in cm)?
- What is your weight (in kg)?
- What is your clothes size (in UK sizing)?
- What is your depth measurement (hip to hip)?
- What is your width measurement (bottom of your back to the back of your knee)?
- Tell us about your mobility. How far can you walk, and with what aids if any?
- Tell us about how you transfer. Do you transfer independently, use a slide or any other aids?
- Do you have any posture concerns?
- Do you have any pressure concerns?
- Are you currently registered with your Local NHS Wheelchair Service? If not, please explain why.
- Do you have a personal wheelchair budget issued by Local NHS Wheelchair Services that you can contribute towards new equipment?
- Do you currently have a wheelchair?

- What is the make and model of your current wheelchair?
- What is its seat size?
- What is the serial number? This is a long number located on a sticker under the chair’s frame.
- Who provided this wheelchair?
- Was this chair acquired through the use of a Personal Wheelchair Budget?
- Tell us about your accommodation, including where you are living and the environment you access.
- Tell us about your university enrolment, including access to buildings, navigating lecture rooms and compulsory placements.
- Do you have a car that your chair must fit into? If not, what is your usual mode of transport?
