This page lists the application form questions and supporting documents you must submit with your grant application. Please read it carefully. Your application will not be processed if evidence is missing. You must complete all sections of the application form.

- Please select the category that best describes your disability, condition, long-term illness, learning difference, or combination of these. Choose one of the following options:

- A learning difference (e.g. dyslexia, dyspraxia, or AD(H)D)
- A visual impairment uncorrected by glasses (e.g. blindness or partial sight)
- A hearing impairment (e.g. D/deafness or partial hearing)
- A physical impairment or challenges with mobility (e.g. climbing stairs or uneven surfaces), or dexterity (e.g. using a keyboard or laboratory equipment)
- A social, behavioural or communication impairment (e.g. an autistic spectrum condition or Tourette’s Syndrome)
- A long-term illness or health condition which may involve pain or cause fatigue, loss of concentration or breathing difficulties – including any effects from taking associated medications.

You must upload the following documents with your application:
- A complete document from an official source that confirms your disability diagnosis. This document must state your diagnosis, be relevant to the grant request you are making, and ideally include information regarding your needs. It could be:
- A letter from your doctor or healthcare provider
- A Disabled Students Allowance Needs Assessment Report (DSA NAR)
- An Educational and Health Care Plan (EHCP)
- A Learning Support Agreement (LSA)
- A mobility assessment or occupational therapy report
- A document from your institution that states you are enrolled on or have a confirmed place on your course. Your student services centre should be able to provide this for you. This document may be called:
- An enrolment letter
- A student status letter
- A certificate of student status
- A copy of your study offer, if you have not yet started your course
- A document that confirms that you are entitled to UK home fee status. This can come from your institution or student finance body. You can find further guidance on fee status here <link>. You can submit a full copy of:
- An entitlement letter from your student finance body
- A fee confirmation or offer letter from your institution that names your fee status
- A document that confirms your fee status for your previous study, if applicable
Section E: Adjustment Type
You will apply under a specific adjustment type category. Select the one which best meets your grant application request to learn more about the information you must submit with your application.
To apply for support for the additional disability-related cost of student accommodation, you should explain:
- Grant request details: why do you need this accessible accommodation, and how will it improve access to your studies as a disabled student?
- Total amount requested [maximum of £5,000]. Please ensure that this amount represents the additional disability-related cost. You can find guidance on how to calculate this amount here.
- If your accommodation is university-owned or privately rented
- If you receive support from your Local Authority or Universal Credit, why this support does not meet the costs you are applying for
You must also upload the following documents with your application form:
- Evidence of your rent amount. If you do not yet have an invoice, you can share an email from your accommodation provider, or a screenshot from an online portal.
- Evidence of the cost of a comparable accommodation which is not accessible to you. This enables us to verify the amount you are requesting. This can be shared directly from an accommodation provider or from an online source.
If either of the following circumstances apply to you, you will also be asked to provide the following:
- I will be staying in university-owned accommodation: Evidence that you have asked your institution to waive any additional accommodation cost as part of their legal obligation to provide you with reasonable adjustments under the Equality Act 2010, and their confirmation that they will not do this. This will usually be a letter or email from them.
- My university does own accommodation, however they cannot provide an accessible solution for me. For this reason, I will be staying in privately rented accommodation: Evidence that you have requested funding from Disabled Students Allowance (DSA) to cover this additional accommodation cost. DSA can fund privately rented accommodation if your university cannot offer you accessible accommodation themselves. This may be included in your Needs Assessment Report, or can be provided by letter.
To apply for support for the cost of accommodation for a Personal Assistant or Carer, you should explain:
- Grant request details: why do you need this accommodation, and how will it improve access to your studies as a disabled student?
- Total amount requested [maximum of £5,000]
- If you receive support from your Local Authority or Universal Credit, why this support does not meet the costs you are applying for
You must upload the following documents with your application form:
- Evidence that you have asked your institution to waive any additional accommodation cost as part of their legal obligation to provide you with reasonable adjustments under the Equality Act 2010, and their confirmation that they will not do this. This will usually be a letter or email from them.
- Evidence of your rent amount. If you do not yet have an invoice, you can share an email from your accommodation provider, or a screenshot from an online portal. This document must demonstrate that you will be leasing 2+ rooms.
To apply for support with disability-related travel costs, you should explain:
- Grant request details: what travel costs you are applying for, and how will this travel improve access to your studies as a disabled student?
- Total amount requested [maximum of £5,000]. Please ensure that this amount represents the additional disability-related cost. You can find guidance on how to calculate this amount here.
- If you receive Personal Independence Payments (mobility element) or your Local Authority provides travel funding, why this support does not meet the travel costs you are applying for
You must upload the following documents with your application form:
- Evidence that you have requested funding for your additional travel costs from your institution as part of their legal obligation to provide you with reasonable adjustments under the Equality Act 2010, and their confirmation that they will not fund this.
- Evidence that you have requested this support from Disabled Students Allowance, and that they are not able to fund your additional travel costs.
- Evidence of the cost of your travel. This can be quotes from a company or fares from an online source.
- Evidence of the cost of the equivalent journey by public transport. This enables us to verify your grant request amount.
To apply for support for equipment, you should explain:
- Grant request details: what equipment do you need and how will it improve access to your studies as a disabled student?
- Total amount requested [maximum of £5,000]
- If you receive support from your Local Authority for mobility or care needs, why this support does not meet the costs you are applying for
You must upload the following documents with your application form:
- Evidence that you have requested this support from your institution as part of their legal obligation to provide you with reasonable adjustments under the Equality Act 2010, and their confirmation that they will not do provide this.
- Evidence that you have requested this support from Disabled Students Allowance, and that they are not able to fund your equipment costs.
- Evidence of the cost of your equipment. This can be a direct quote or prices available from online sources
To apply for support for support for the cost of human support, you should explain:
- Grant request details: why do you need this support, and how will it improve access to your studies as a disabled student?
- Total amount requested [maximum of £5,000]
- If you have received NMH support in previous years, how many hours of support did you use? Please provide details and costs if known
- Which band does this support come under? You can find guidance on this here.
- My support comes under Band 1 or Band 2 (with the exception of Sighted Guide support): Please provide evidence that you have requested this support from your institution as part of their legal obligation to provide you with reasonable adjustments under the Equality Act 2010, and their confirmation that they will not do this.
- My support comes under Band 3 or Band 4, or I need Sighted Guide support: Please provide evidence that you have requested this support from Disabled Students Allowance, and that they are not able to fund your support costs, or if you are receiving DSA but your support costs exceed the maximum allowance, please provide your complete DSA Needs Assessment Report.
- If you receive support from your Local Authority for mobility or care needs, or Personal Independence Payment, why this support does not meet the costs you are applying for
You must also upload the following documents with your application form:
- Evidence of the cost of the NMH support you are requesting. This can be a direct quote or prices available from online sources.
To apply for support for mobility equipment, you should explain:
- Grant request details, including what support you need and why you need it
- Specific information in response to questions about your mobility, environment and any current equipment. You can find these questions here.
You will also need to provide the following:
- Evidence that you have requested this support from your Local NHS Wheelchair Service, and that they are not able to fund your equipment costs. This will usually be a letter or email from them.
